To expedite the process, please have the following ready when you come for green card visits. Listed below are the Green Card examination requirements.
Green card for children (0-11)
- Summary of health record/last Comprehensive Physical Examination
- Routine childhood vaccines
- 2 yrs and up (PPD) and CXR (If PPD Positive)
Green Card Adults (12 & up)
- Summary of health record from primary care physician
- Vaccination records
- Tetanus
- MMR
- Varicella
- PPD and CXR (If PPD Positive)
- RPR test
- Complete USCIS form 691 (USCIS website)
Please note:
- The green card evaluation is a 2 day process. Health insurance does not cover green card visits.
- Physical exam ; Evaluation for STD, Tuberculosis, Mental Illness
- Tuberculosis testing (PPD) required for paper work for ages 2 and older.
- If tuberculosis test comes out positive, chest x-ray is required
- Children ages sixteen and younger need routine childhood vaccines.
- Government required vaccines – MMR, Tetanus, Varicella.
(Or titer levels) - Pneumococcal vaccine is required for ages 65 and older.
