Tajudeen O. Fawole, MD is board certified in Internal Medcine, Geriatric Medicine and Hospice/Palliative Medicine. Dr. Fawole completed his Internal Medicine Residency at Interfaith Medical Center, Brooklyn, New York and Fellowship in Geriatic Medicine at New York Medical College, Our Lady of Mercy Hospital in Bronx, New York. Dr. Fawole has worked in many hospitals including the Interfaith Medical Center in Brooklyn, New York; Fairview Health Care System, Minnesota; Regency Hospital of Minneapolis and Minneapolis VA Health Care systems. Dr. Fawole’s area of expertise includes but is not limited to: hypertension, diabetes, adult immunisation and COPD. Dr. Fawole is a member of American College of Physicians and the American Geriatric Society. He has been a practicing physician for more than 20 years and accepts most private insurances and Minnesota Medicaid.

Dr. Abejide (Abe) Fawole DNP, RN, CNP is board certified Geriatric Nurse Practitioner. She received her master’s degree in nursing and Doctor of Nursing Practice degree from the University of Minnesota. She has worked with Walker Methodist Health Care Center in Minneapolis, St Joseph Hospital in St. Paul, Minnesota and currently works as a Geriatric Nurse Practitioner with HealthPartners. Dr. Fawole has over 20 years of professional nursing experience in managing acute and chronic medical problems in adult and geriatric populations. Dr. Fawole is a primary care provider, educator and advanced care coordinator in sub acute care settings, transitional care units, nursing homes, and assisted living.
